North American Floorball League, LLC Participation Waiver

This Release is signed by the undersigned participant (herein referred to as "Participant"), in favor of the North American Floorball League, LLC, its franchise teams, their respective affiliates, officers, directors, employees, agents, and event sponsors (collectively, "NAFL Released Parties"). If the Participant is a minor, their parent or legal guardian must sign this Waiver. This Waiver covers all activities, including practices, exhibitions, games, tournaments, and any other appearances or activities (collectively “Activities”).

1. Release of All Claims:
Participant hereby releases, waives, discharges, and covenants not to sue the NAFL Released Parties from all liabilities, claims, actions, damages, costs or expenses of any nature whatsoever, whether in law or equity, known or unknown, occurring during or as a result of participating in the Activities, including those which may be attributable to the negligent acts or omissions of the NAFL Released Parties. This release includes any claims of negligence by the NAFL Released Parties and is intended to be as broad and inclusive as is permitted by the laws of the State of Texas or the laws of any other state.

2. Risks Accepted / Emergency Medical Treatment:
By participating in Activities, Participant understands there is a risk of physical injury and Participant assumes all such risks. Participant is responsible for their own medical costs. NAFL Released Parties may provide emergency medical assistance but are not obligated to cover medical costs.

2. Communicable Diseases Notice:
Participant understands that there is a risk of exposure to communicable diseases and accepts this risk. Participant agrees to comply with all health protocols as may be required by public health authorities or by the league.

4. Grant of Publicity Rights:
Participant grants NAFL Released Parties the right to use Participant's name, image, and likeness for publicity and promotional purposes without compensation.

5. Indemnification:
Participant agrees to indemnify and hold harmless the NAFL Released Parties from any liabilities, losses, claims, or expenses arising from their participation in Activities.

6. Severability:
If any provision of this waiver is found to be invalid or unenforceable, it shall be deemed replaced by provision that is valid and enforceable and that reflects the intent of the parties as expressed in this Waiver, and the remaining sections of this Waiver will remain effective.

7. Dispute Resolution:
If there is a dispute regarding any portion of this Waiver, it will be settled through binding arbitration before a single arbitrator in accordance with the rules of the American Arbitration Association using its Expedited Procedures, conducted in Texas.

8. Insurance Requirement:
Participants are required to maintain their own health insurance to cover personal injuries. Secondary Insurance coverage may be available through USA Floorball or other sources but is not guaranteed by NAFL Released Parties.

9. Acknowledgment of Understanding:
By signing, the Participant confirms understanding and voluntary agreement to the terms of this Waiver.

10. Data Privacy and Protection:
Participant information will be used and protected in accordance with applicable privacy laws.
